Getting Help Resource Request Forms

The University of Minnesota Supercomputing Institute for Digital Simulation and Advanced Computation offers user support, access to laboratories, supercomputer resource allocations, research scholarships, and travel awards. A single form now suffices to obtain user support, access to laboratories, and/or supercomputer resource allocations, but there are three versions of this form, depending on whether or not one already has an account and whether or not one already has a supercomputer resource allocation in a given six-month period.

Which form do I use?

If you are a group member, but not a principal investigator (i.e., group leader), do not use these forms directly. You may, however, assist the faculty member who leads your research group to submit the proper form. The rest of this explanation is directed to faculty principal investigators or potential faculty principal investigators.

Forms are available either as PDF files (requiring Adobe Reader) or RTF files (suitable for word processing applications).

New Project Form

This form is for initiating new projects at the Institute. Return the form to the Research Programs Administrator by email or to the address on the form.

Project Renewal Form

Once you have a project registered or laboratory accounts for yourself and/or your group or a supercomputing resource grant, you need to renew your registration and/or grant every six months. This involves filling out the first half of this form for user support and/or laboratory resources or the whole form (that is, include a proposal) if you also request supercomputing resources. Return the form to the Research Programs Administrator by email or to the address on the form.

Additional Supercomputing Resources Form

Use this form if you already have a Supercomputing Resources grant for the current six-month period (January–June or July–December) but you need to request a supplemental supercomputing resources grant for this period. Return the form to the Research Programs Administrator by email or to the address on the form.

Add Group Member Form

Use this form if you have a project registered and/or laboratory accounts for yourself and/or your group or if you have a supercomputing resource grant, and you wish to add another person to your group. Return the form to Accounts Database Manager by email or to the address on the form.
